The Wheaton Wings Soccer Club is pleased to be hosting the inaugural Wheaton Wings Spring Classic tournament. We appreciate all of the support we have received in the local community and the greater soccer community.

Registration
Registration for the 2012 Wheaton Wings Spring Classic is officially closed.
If your team has been accepted, you can find specific tournament information below.
Paperwork
All paperwork needs to be submitted by May 4, 2012
Submit paperwork in one of three ways:

All teams need to submit the following forms:
- Official Team Roster – All teams will be required to submit a copy of their approved US Club Rosters or their USYSA roster with state add/drop forms (as needed)
- Official Tournament Roster - All teams will be required to submit a copy of their signed IYSA Sanctioned Tournament Roster Form. Only the players participating in the event should be listed. Your tournament roster may be changed up to final check in. At this point it is final and cannot be altered for the tournament.
- Guest Player Forms - Up to three guest players may be added to a team roster. USYSA teams with guest players must submit an IYSA/USYSA tournament guest player permit form. If the player is from the same club, no form is needed. The player must only be added to your Tournament Roster. US Club players must be on your official US Club Roster.
- IYSA/US Club Medical Release Forms – Teams may submit either the US Club Soccer or the IYSA Medical Release Form for each player.
- Permission to Travel Form (All non-Illinois teams) USYSA teams from outside Illinois must submit an approved “Permission to Travel” form from their home state’s USYSA affiliate.US Club teams are not required to have a travel permit.
- IYSA Tournament Team Agreement (non US Youth Soccer and foreign teams) All non-IYSA (including U.S. Club teams) and foreign teams must submit a completed IYSA Tournament Team Agreement (Form NT) (available on website). All foreign teams must also provide a completed form from their Provincial or National Association approving the team’s participation in the tournament and permission to travel.

Onsite Registration Sessions
Thursday, May 17, 2012 from 5:30-9pm at the Wheaton Park District Community Center, 1777 S. Blanchard, Wheaton, IL 60189.

Friday, May 18, 2012 from 4-8pm at Graf Park, 1855 Manchester Rd., Wheaton, IL 60187 under the Tournament Headquarters tent.
Please note: All teams are required to check-in on Thursday, May 17 from 5:30–9 pm at Wheaton Park District Community Center, 1777 S. Blanchard, Wheaton, IL 60189. If you cannot attend that evening you may check in at the Tournament Headquarters Tent on Friday, May 18 from 4-8 pm. Coaches must register no later than 1 hour prior to a game. You will not be able to finalize registration without the required paperwork (See paperwork section above.) At the time of registration, these additional items/information will be required.
- Laminated Coach/Player Passes (hole punched to allow for cable tie) All rostered players and coaches must have a current USYSA or US Club Soccer Player Pass.
- Emergency Contact Information for the coach or team manager while attending the tournament.
Referees
All U11 and older games will use a three referee system with all referees being USSF certified. Referees can email Don Immekus at dimmekus@sbcglobal.net for more information.
