Download and complete a PDF form below to submit your evaluation electronically:
NOTE: Please read all of the following if you have trouble!
Directions: Click on link above to open the PDF form. You will likely see something that looks like this in a new window:
Some browsers will not allow you to see/click on fields to edit a PDF form online. Some files will give you no trouble in one browser but very uncooperative in another. If you find that this is the case for you--you can do a few things:
- Open the file in Adobe Acrobat (the ideal option; there's a couple of ways to do this)
- Download a spreadsheet here and complete the form in Excel
Click this button if you have trouble in your web browser with the file and need to open it in Adobe Acrobat.
Save or Open the file
Click this icon to save the file to your computer.
To open the file in Adobe Acrobat, you can:
Look for a button or command that's called Open With Different Viewer (the prompt is likely in the upper right hand corner of the window).
Once you do that, you will be asked if you would like to Open the file or Save it. Opening the file in Adobe Acrobat means you won't have to locate the file on your machine to open it in an additional step.
If you prefer to Save the file, you can go through this process and select Save File or you can look for the download icon (circled in red at left).
Additionally, you can also save the file to your computer by selecting Save, Save As, or Save Page As from your browser's main menu.
After opening the file in Acrobat, you should be able to click on and add data to the fields. Tip: the tab button will allow you to skip to the next field without having to click on it.
Ready to submit? Click the Email your completed form button at the bottom of the page. This will likely prompt your computer/Acrobat to ask what email program you would like to use to send the file. If you have trouble, select Save As from the Acrobat menu and save your completed form so you won't lose all that data. Then, you can email firstname.lastname@example.org and attach the file to your message.
Having trouble or don't have email set up on your computer? Email email@example.com or try the Excel spreadsheet file below.
